Ampla Health provides outpatient services in our Anticoagulation Clinic to help patients manage their anticoagulation medications [sometimes referred to as ‘blood thinners’]. These medications, such as Coumadin and low molecular weight heparin, are prescribed for people at risk to develop blood clots from irregular heart rhythms, heart attacks, strokes, and other conditions.
The Anticoagulation Clinic is staffed with a Chief Medical Officer, a nurse practitioner, and specially trained nurses who work with patients and their physicians to monitor and adjust their medication dosage. The anticoagulation team also provides education and support to help patients manage changes in diet, medicines, and other factors that can interact with the medication. Studies show that patients managed by an Anticoagulation Team experience fewer adverse events, including bleeding and blood clots, as well as fewer hospital visits resulting from these complications.
